Leave Application for Office: 15 Templates, Free Generator and US Notice Rules (2026)

How to write a leave application for office: a free generator, 15 copy ready email templates including FMLA, pregnancy and staffing contractor requests, notice periods, manager replies and the leave rules in the US, UK, Australia and India.

Quick answer: A good leave application fits in five sentences: the leave type, exact dates with your return day, a one line reason, who covers your work, and how to reach you. Send it to your direct manager, copy HR if your company asks for it, and do not start the leave until you have a written yes. In the US, leave that may fall under the FMLA needs 30 days of notice when you can see it coming, and the same or next business day when you cannot.

What you get here: a free leave application generator, 15 copy ready templates (including FMLA, pregnancy, disability, caregiver and staffing contractor versions), a notice period table, manager reply scripts, an AI prompt, and the rules in the US, UK, Australia and India.

What is a leave application for office?

A leave application is a written request to your employer for time away from work, with the dates, the type of leave and a plan for your work while you are out. Today it is almost always an email, a message in an HR system, or both. The written version matters because it becomes the record your manager uses to plan coverage and payroll uses to code the absence as paid time off, sick leave or unpaid leave.

In the US, there is no single federal rule for everyday time off. The FLSA vacation rules say federal wage law does not require pay for vacations, sick days or holidays; those are matters of agreement between employer and employee. That means your company handbook decides most of the process, and the law steps in for specific situations such as FMLA leave, disability, pregnancy and state paid sick leave.

What types of leave can I ask for?

Eight kinds of leave cover most US requests. The tag on each card shows what sets the rules: federal law, state law or your company's policy. Use the matching type in your request so payroll codes it correctly.

Vacation or PTO Company policy

Legal basis
No federal requirement. The FLSA leaves paid vacation to your employer.
How it works
Accrues each pay period or comes as a yearly bank, as your handbook sets out.
Request tip
Ask two to four weeks ahead and name who covers your work.

Sick leave State law

Legal basis
Paid sick leave is required in about 20 states and DC, not by federal law.
Notice
Tell your manager before your shift, or as soon as practicable.
Doctor's note
Allowed only when the rule applies to everyone in the same situation.

FMLA leave Federal law

Entitlement
Up to 12 workweeks a year, unpaid and job protected, with health coverage kept.
Who qualifies
12 months and 1,250 hours worked, at a site with 50 employees within 75 miles.
Request tip
Give 30 days notice when you can. You do not have to name the law.

Parental leave Federal and state

Legal basis
FMLA covers birth, adoption and foster placement for eligible employees.
Pay
Set by your employer and, in some states, a state paid family leave program.
Request tip
Taking bonding time in separate blocks needs your employer's agreement.

Pregnancy related leave Federal law

Legal basis
The Pregnant Workers Fairness Act can require leave as a reasonable accommodation.
Who qualifies
Separate from FMLA, so it can apply even when FMLA does not.
Request tip
Describe the limitation and what you need, not your full medical history.

Medical or disability leave Federal law

Legal basis
The ADA can require unpaid leave as an accommodation at employers with 15 or more employees.
Limits
Indefinite leave with no expected return date does not have to be granted.
Request tip
Give an expected return date, even an approximate one.

Bereavement leave Company policy

Legal basis
No federal requirement. A few states have their own rules, so check yours.
Typical use
The funeral, travel and family matters after a death.
Request tip
Name your relationship to the person. HR may need it for its records.

Jury or military duty Federal and state

Military
USERRA protects your job while you serve and when you return.
Jury
Federal law protects jurors in federal court. State law covers state courts.
Request tip
Attach the summons or orders and give notice as early as you can.

What should a leave application include?

Six parts, and most can be one sentence each. Requests get stuck when the reader has to write back and ask for something that should have been in the first message, usually the return date or who is covering.

Subject line

Leave type, your name and the dates, so it can be read without opening.

ExampleVacation request: Jordan Lee, June 24 to 26

Greeting

Your direct manager by name. Copy HR in the same email if your company asks for it.

ExampleHi Priya,

The request

Type of leave, exact dates, working days and your return day, all in one sentence.

ExampleI would like to request vacation from Wednesday, June 24 through Friday, June 26 (3 working days). I will be back Monday, June 29.

Reason

One line, or none at all for vacation and personal days.

Examplefor a family wedding out of state

Coverage

Who handles urgent items, and the status of anything with a deadline.

ExampleSam will cover client calls. The Q2 report goes out Tuesday.

Close

How reachable you will be, then a thank you.

ExampleI will be offline but Sam can reach me for anything critical. Thanks, Jordan

Six parts, one sentence each. Miss one and your manager has to write back before approving.

For a medical reason, give the nature of the need, not the diagnosis. "I have a planned procedure and my doctor has advised two weeks of recovery" is enough to start the process. If the leave might be covered by the FMLA, the regulations say you do not have to name the law in your first request, but you do need to give enough information for your employer to see that it may apply, and you should answer their follow up questions.

How much notice should I give for time off?

Follow your handbook first. The table below shows where the law sets a floor and where it is simply good practice. Under the FMLA notice rule, an employee who learns of a need for leave less than 30 days ahead should normally be able to give notice the same day or the next business day. For sudden needs, the unforeseeable leave rule asks for notice as soon as practicable and lets a spouse or family member call in if you cannot.

SituationNotice to giveWhere it comes from
Vacation or personal day, 1 to 3 daysWhatever your policy says; two weeks is a safe defaultCompany policy (US federal law sets none)
Vacation of a week or moreFour weeks or more, earlier for peak seasonsCompany policy; Work Insiders guidance
Sick dayBefore your shift starts, using the call in method your employer namesCompany policy; state sick leave laws where they apply
Foreseeable FMLA leave (birth, adoption, planned treatment)At least 30 days, or the same or next business day once you know29 CFR 825.302
Unforeseeable FMLA leaveAs soon as practicable, following the usual call in procedure29 CFR 825.303
UK annual leaveTwice the length of the leave plus 1 day, unless the contract says otherwiseGOV.UK
Australia sick or carer's leaveAs soon as practicable, which can be after the leave startsFair Work Ombudsman

Timing also changes the outcome. The FMLA rules let an employer delay or deny protected leave when an employee skips the usual notice procedure without an unusual reason, which is why it pays to learn your company's process before you need it.

Do I have to give a reason for taking leave?

For vacation and personal days, usually not. Many companies treat PTO as time you have earned, and "I would like to request a personal day on Friday" is a complete request. For sick leave, a short statement that you are unwell is normal. For longer medical, family or disability leave, you will need to share enough for your employer to decide which rules apply, and they may ask for a doctor's certification.

Documentation rules must be applied evenly. The EEOC leave guidance gives the example of a supervisor who demands a psychiatrist's note from one employee when the company policy asks no one else for documentation, and says that violates the ADA. Employers can require a doctor's note, but only if the rule applies to everyone in the same situation.

Which leave laws apply to me in the US?

It depends on your employer's size, your state and the reason for the leave. Most leave requests sit under company policy alone; the laws below cover the bigger situations, and they can overlap.

LawWho it coversWhat it givesWhat to put in your request
FMLAPrivate employers with 50+ employees, all public agencies; you need 12 months, 1,250 hours and 50 employees within 75 milesUp to 12 workweeks of unpaid, job protected leave a year, with group health coverage keptThe reason in plain words, expected dates and duration
ADAEmployers with 15+ employeesUnpaid leave as a reasonable accommodation for a disability unless it causes undue hardshipThat you need leave for a medical condition and roughly how long
Pregnant Workers Fairness ActCovered employers, separate from FMLA eligibilityReasonable accommodations, which can include leave, for pregnancy, childbirth and related conditionsThe pregnancy related limitation and the leave or change you need
State paid sick leaveDepends on the state; many have employer size thresholdsPaid sick time that accrues as you workThat you are using sick leave and the dates
USERRAAlmost all employersJob protection for military serviceAdvance notice of service dates, written or verbal

The details matter. The DOL FMLA page sets out the 50 employee threshold and the 12 month, 1,250 hour eligibility test. The EEOC says the ADA can require unpaid leave even when an employee is not eligible under company policy or has used up FMLA leave. And under the PWFA guidance, leave can be a reasonable accommodation for pregnancy. The EEOC sued a Florida call center after it allegedly denied pregnant employees' leave requests only because they did not qualify for FMLA, without considering the PWFA, according to its EEOC lawsuit announcement.

Paid sick leave is a state patchwork. A CRS report from April 2026 says federal law generally does not give private sector employees paid sick leave, while 18 states including DC require it and 3 more require paid leave usable for any purpose. Military members can read the USERRA overview from DOL.

How many US workers actually get paid sick leave?

Most, but not all, and it depends heavily on your job and region. According to BLS paid sick leave data, 80% of private industry workers had access in 2025, ranging from 98% in the Pacific division to 63% in the East South Central states. CRS reports a similar gap by occupation: 93% of management and professional workers versus 65% in service jobs. If you are in a group with lower access, check your offer letter before you ask, because your request may be for unpaid time.

How do I ask for leave on Slack or Teams?

Use chat for the heads up and the official channel for the record. A quick message works for a sick day ("Hi Priya, I'm sick today and offline. Sam has my 2pm call. Formal request is in Workday."), but most payroll and leave records still come from your HR system or email. If your company only uses chat, send the request as a direct message with the same six parts and keep the approval.

What if my manager does not reply?

Follow up once after one or two working days, in the same thread so the original is visible: "Hi Priya, following up on my vacation request for June 24 to 26 below. Happy to adjust anything." If you still hear nothing and the dates are close, ask HR how approvals work when a manager is away. Do not treat silence as a yes, especially for unpaid time.

How should managers reply to a leave request?

Fast, in writing, and with the dates repeated back. Recruiters and HR teams also send these on behalf of hiring managers and clients.

ApproveHi Jordan, approved for June 24 to 26, back June 29. Thanks for setting Sam up on the client calls. Enjoy the time off.
Approve with a conditionApproved. Could you send the Q2 report to the client before you leave on the 23rd?
Ask to move datesThanks for asking early. June 24 overlaps the audit. Could you take July 1 to 3 instead? If not, let's find a fix together.
Medical or family leaveThanks for letting me know, and I hope everything goes well. I've copied HR so they can walk you through the next steps and any forms.

One caution for managers: a request that mentions a health condition, pregnancy or a family member's serious illness may trigger FMLA, ADA or PWFA duties even when the employee never names a law. Pass it to HR the same day rather than deciding alone. HR teams handling many requests usually track them in leave management software or an HRIS instead of inboxes.

Who do I ask if I work through a staffing agency?

Both your agency and the client manager you report to, in the same message. The agency usually runs your timesheets and pay, while the client runs the day to day work, so telling only one of them leaves a gap. For FMLA, the DOL explains that two businesses can jointly employ the same worker, with the staffing agency usually acting as the primary employer; its joint employment fact sheet covers who handles notices and job restoration. Template 15 above is written for this case, and our guide to staffing agency software covers the tools agencies use to log contractor time.

Can AI write my leave request?

Yes, and it is a good use of it, as long as you check the dates and remove anything private before you send. Paste this into ChatGPT, Claude or Gemini:

Copy ready prompt

Write a short, professional leave request email to my manager [name]. Leave type: [vacation / sick / personal / medical / parental]. Dates: [first day] to [last day], back on [return date]. Reason in one line: [reason or "no reason"]. Coverage: [colleague] will handle [tasks]. Status: [deadlines]. Tone: [warm / formal]. Include a subject line with my name and the dates. Keep it under 120 words, do not include any medical diagnosis, and do not invent details I have not given you.

How do leave rules differ in the UK, Australia and India?

If you work for a global team, the minimums change by country, and they shape how much notice your employer can ask for.

CountryPaid annual leave floorSick leave floorNotice rule worth knowing
United StatesNone under federal lawNone federally; set by state law in about 20 states and DCFMLA: 30 days for foreseeable leave
United Kingdom5.6 weeks for most workersStatutory Sick Pay for eligible workersNotice of twice the leave length plus 1 day
Australia4 weeks for full time employees10 days paid sick and carer's leave a yearSick leave notice as soon as practicable
IndiaSet by the Factories Act and state lawsSet by state Shops and Establishments Acts26 weeks paid maternity leave for the first two children

For the UK, see the GOV.UK holiday rules, which also say an employer that refuses leave must give notice equal to the leave requested plus 1 day. For Australia, the Fair Work fact sheet sets the 10 day sick and carer's leave entitlement. In India, maternity leave runs under the Maternity Benefit Act.

How we built this guide

We reviewed the leading leave application guides, including HR Stacks, Naukri, Calm and AttendanceBot, and kept what they agree on: a dated subject line, a clear return date and a coverage plan. Then we added what most of them leave out for US readers: the federal notice rules in 29 CFR 825.302 and 825.303, ADA and PWFA leave, state paid sick leave counts, staffing contractor requests and manager replies. Legal points come from the DOL, EEOC, eCFR, BLS, CRS, GOV.UK and Fair Work, checked on October 14, 2026. The templates and generator are Work Insiders originals. Leave terms are defined in our recruiting glossary.

Quick answers

How do I write a simple leave application for one day?
State the date, the type of leave, who covers urgent work, and thank your manager. Template 3 above does it in three sentences.

What is the best subject line for a leave request?
Leave type, your name and the dates, for example "Vacation request: Jordan Lee, June 24 to 26". Your manager can see everything without opening it and find it later by searching.

Can my employer deny my vacation request?
In the US, usually yes, because federal law does not require paid vacation and the timing is set by company policy. Leave protected by laws such as the FMLA, ADA or state sick leave rules works differently.

Do I have to say "FMLA" in my request?
Not the first time. The regulations say you need not mention the FMLA, but you must give enough information for your employer to see it may apply. For a repeat need, you must reference the reason or the FMLA.

Is calling in "sick" enough notice for FMLA?
No. Under 29 CFR 825.303, calling in sick without more information does not trigger FMLA obligations, so say briefly why you need the time.

Can my boss ask for a doctor's note?
Yes, if the policy applies to everyone in the same situation. The EEOC says requiring a note from one employee with a disability when others are not asked violates the ADA.

What if I do not qualify for FMLA?
You may still have rights. The ADA can require unpaid leave as an accommodation, the PWFA covers pregnancy related leave, and your state may give paid sick time.

Should I offer to be reachable on vacation?
Only if you mean it. A clear line such as "I will be offline, and Sam can reach me for anything critical" is better than an open offer you will regret.

The bottom line

Write it like you would want to receive it: exact dates, return day, one line of reason, a named backup, and a clear note on how reachable you will be. Use the generator or a template above, send it early, and keep the written approval.

For anything medical, family or pregnancy related in the US, give your employer enough detail to see which rules apply and loop in HR the same day. When the stakes are high, check your state law or speak to employment counsel.
